How to complete a Personal Financial Statement (PFS) form?

1. For best results (on both PC and Mac), download Adobe Acrobat Reader if it is not already on your computer. You can download it for free here.

2. Download your Personal Financial Statement Template from the application. 
   
   
   
   
3. Fill out the template. Enter each asset and liability and they will total in the summary at the bottom.

4. Save the file in your computer.

5. Click Upload A File and upload your saved template. Click Mark As Complete once you're done.
