# How to complete a Personal Financial Statement (PFS) form?

## Learn how to complete and submit a PFS form PDF through Panacea Financial, including downloading, editing, and submitting for review.

 

1. For best results (on both PC and Mac), download Adobe Acrobat Reader if it is not already on your computer. You can download it for free [**here**](https://get.adobe.com/reader/).
2. Download your **Personal Financial Statement** Template from the application.

3. Fill out the template. Enter each asset and liability and they will total in the summary at the bottom.

4\. Save the file in your computer.

5\. Click **Upload A File** and upload your saved template. Click **Mark As Complete** once you're done.
